10. Briefly, your reason for leaving your previous guild/server. Bluebriar reccomended I switch after trial account expired.

11. How many days a week/hours a day would you like to raid? Once leveled I'll try to be on whenever work permits.

That it's an established mature guild. It was reccomended by Bluebriar who I work with.
